Health Center Controlled Network Project

In August 2016, the Coalition of Orange County Community Health Centers was awarded a federal grant and the designation of Health Controlled Network (HCCN) for 10 of it’s 26 members by the Health Resource and Service Agency (HRSA) of the federal government; and has since expanded to a total cohort size of 12 members, and further expansion within Orange County (and possibly in adjacent counties) is anticipated.

HCCN’s are a common approach to bring Federally Qualified Health Centers (FQHC’s), and Look-a-likes, together for the purpose of strengthening quality of care and improving patient health outcomes by achieving meaningful use of Office of the National Coordinator (ONC)- certified electronic health records (EHR), adopting technology and quality improvement strategies, and engaging in health information exchange (HIE).
